  • Service Form (PDF): required for all services, print and bring with you or complete on site when you arrive for your appointment

  • Sorting Questionnaire: required for cell sorting services, must be completed and returned to our office at least 24 hours before your scheduled cell sorting appointment

  • FACS Analysis Checklist: required for cell analysis services, review guidelines listed for bringing materials for flow cytometry

  • Sample Submission for Flow Cytometry: required for cell analysis services, requirements for packing and shipping samples for flow cytometry
